Badge applications MUST be submitted by your Authorized Signer before proceeding with fingerprinting (if required).

All applicants are required to present two (2) acceptable and unexpired forms of identification with the same spelling and placement of names. Please review the Acceptable Documents (OAK I-9) form above for a list of acceptable documents.

Appointments for fingerprinting and computer training fill up quickly. Please login to the OAK Badging Office schedule as early as possible to ensure you receive or renew your badge by your preferred date (for renewals, up to 30 days prior to your current badge expiring).
