Renewal Applicant Checklist
Expired Badge
A new application must be submitted. Go to the New Applicant Checklist.
Renewal
- Authorized Signer submits a renewal application.
An email from the “OAK ID Badging Office” will be sent to the Authorized Signer and applicant after submission.
The Authorized Signer will verify that the applicant has two unexpired acceptable government issued identifications which MUST be provided at all appointments. Both IDs should have identical spelling and placement of applicant’s name. Please review the OAK I-9 form guidelines.* If Customs Clearance is needed, applicant must provide the CBP packet at time of Fingerprint Appointment.
- Schedule your renewal appointment.
ID Badging Office Appointment Scheduler (also provided in your email).
Scheduled appointment must be made within 30 days of application submittal. - Applicant completes and signs forms.
The email from the “OAK ID Badging Office” contains links to forms required to be completed and signed before their appointment. - Arrive on time to your appointment and provide your two (2) pieces of identification.
Those arriving more than 15 minutes late or without proper IDs may be asked to reschedule the appointment. - Payment is due at your appointment:
Badge Fee: $25

- Check
- Credit/Debit Card (Mastercard, Visa, Discover, American Express)
- Invoice (If your company has an invoice account with OAK)
